I am homebound, how can I get vaccinated?

If you are a homebound individual, you can schedule a vaccination appointment here by filling out the Homebound Intake Form. After filling out the form, someone in your town will be in touch. Your information will be shared with that person only. They will make sure you get your COVID-19 shot.
